Practice Facilitators

Adelaide PHN is seeking skilled, motivated and enthusiastic candidates for 5 new positions of Practice Facilitator to support general practice in our Adelaide metropolitan region.

The primary role of the Practice Facilitator is to support Primary Health Care service providers, particularly General Practice to deliver high quality, safe, evidence-based care to their communities. The Practice Facilitators will achieve this through active communication, engagement, developing effective working relationships with health care providers and hands on coaching and facilitation of quality improvement activities and programs.

Extensive experience in Quality Improvement and practice improvement will be expected of the successful applicants. Additional experience in areas such as organisational development and or project management would be an advantage.

The successful applicants will hold a Certificate IV in Training and Assessment and/or have experience in working with general practice and supporting care teams and practice teams to learn and improve the quality of primary care.

General Practice Liaison Officer (GPLO) - NALHN General Practice Liaison Unit
0.6 FTE Casual Contract to 30 June 2019 

Are you a dynamic and community-minded general practitioner? Are you looking for a collaborative role genuinely tasked with driving positive change and improving health outcomes across the acute/primary care sector?

Adelaide Primary Health Network (APHN) is an independent, not for profit, primary health care organisation. We aim to improve the health and wellbeing of the Adelaide community by coordinating and integrating health and community services and by working in partnership to identify local needs and create strategies in response to keeping people well and out of hospital.

The General Practice Liaison Unit (GPLU) is a joint collaboration between Adelaide PHN (APHN), and the and the Northern Adelaide Local Health Network (NALHN) and is located in the Northern metropolitan region.

The GPLU is tasked with improving the care of patients with chronic conditions by facilitating and strengthening collaboration, communication and integration between general practice and the Lyell McEwin Hospital.

As part of the establishment of the GPLU, Adelaide PHN is seeking a professional, motivated and enthusiastic candidate for the position of General Practitioner Liaison Officer (GPLO). The successful applicant must be a currently practicing General Practitioner with experience leading and managing a team to achieve outcomes across the acute/primary care sector.

The GPLO role will involve:

  • Engagement and relationship building between the NALHN clinicians and general practice.
  • Working with General Practitioners to improve the quality of clinical handover from the community to the NALHN. 
  • Working with the NALHN inpatient services to improve the quality and timeliness of clinical handover on discharge back to the community.
  • Working with the NALHN as part of the outpatient reform process to reduce outpatient waiting lists and improve access to care.  
  • Contributing to newsletters and other forms of communication.

At a minimum, the successful candidate must have a demonstrated knowledge and understanding of chronic condition management in the primary care setting and of administrative policies and practices of a large public hospital. You must also be passionate about and committed to driving positive change throughout primary care and improving health outcomes.

This work will be situated in the GPLU and will be based offsite within the Northern metropolitan region. The GPLU team will also include a care coordinator and administrative support and will commence in July 2018.

Quality Improvement Facilitators (Palliative Care)

2x 1 FTE contract positions to 30 June 2020 available

Do you have a genuine passion for end of life care in Adelaide?  Are you excited by the prospect of working with residential aged care facilities to improve quality and choice?

Adelaide Primary Health Network (APHN) is an independent, not for profit, primary health care organisation based in Adelaide. We aim to improve the health and wellbeing of the Adelaide community by coordinating and integrating health and community services and by working in partnership to identify local needs and create strategies in response to keep people well and out of hospital.

We have a unique opportunity for two Quality Improvement Facilitators (Palliative Care) to join our Community Collaborations team at the Adelaide PHN.

The Quality Improvement Facilitators (Palliative Care) will be responsible for the implementation, delivery and evaluation of the Enabling Choice for South Australia (ECSA) Project.  ECSA is the Adelaide PHN’s response to the Australian Government’s Greater Choice for at Home Palliative Care measure. The Quality Improvement Facilitators (Palliative Care) will work with aged care organisations to build organisational workforce, resource and partnership capacity to improve the planning and delivery of palliative and end of life care to achieve the following objectives:

  • Improving access to safe, quality palliative care in aged care and support end of life care systems and services in primary and community care;
  • Enabling the right care, at the right time and in the right place to reduce unnecessary hospitalisations;
  • Generating and using data to ensure continuous improvement of services across sectors; and
  • Utilising effective technologies to provide flexible and responsive care, including after usual business hours.

The successful applicant will demonstrate an extensive understanding of not only the Australian aged care sector and the primary and tertiary health sectors but will also have an indepth knowledge of palliative and end of life care.You will also have experience in palliative care service implementation and management, including palliative care capacity building interventions within the aged care and/or community aged sector.

Significant experience in building beneficial professional relationships and working collaboratively with a wide range of stakeholders, including aged care providers, general practice and hospital and community services will be expected of the successful applicant.
