Palliative Care QI Adelaide PHN - An Australian Government Initiative

Palliative Care QI

Adelaide PHN has successfully secured Commonwealth funding through the Greater Choice for At Home Palliative Care measure.  These funds are part of the Commonwealth’s increasing commitment to the palliative care workforce and sector, and represent $8.3 million over three years (from 2017-18 to 2019-20) through 10 PHNs.

This project is a capacity building project and focusses on residential aged care facilities.
This project will:

  • deliver Palliative Care Quality Improvement Packages in residential aged care settings through Adelaide PHN's employment of two Quality Improvement Facilitators
  • establish an Intersectoral Collaboration Group to improve integration and collaboration between primary care, aged care, acute and specialist palliative and end-of life-care sectors

The Quality Improvement Facilitators will act as a resource for residential aged care organisations by working with staff to identify and prioritise issues which support person-centred and directed end of life care.

The Intersectoral Collaboration Group will consider issues identified through the project and facilitate pathways across sectors through better relationships.

Learnings will be transferable from residential into the home care setting, as aged care organisations position themselves for the provision of palliative and end of life care across both settings.

Providers seeking more information about the packages and application process are encouraged to attend our upcoming Agency Briefing on the 19th of June 2018. Please register for the briefing via Eventbrite here.
