Adelaide PHN Operational & After Hours Activity Work Plan

The Operational & After Hours Activity Work Plan covers the period from 1 July 2016 to 30 June 2018.  It will identify local priorities which in turn will inform and guide the activities to correspond with the PHN Objectives, Key Priorities and/or the National Headline Performance Indicators. Additionally the After Hours Activity Work Plan address gaps in after hours service provision and improve service integration within the PHN region.


Adelaide PHN Mental Health Activity Work Plan

The Mental Health Activity Work Plan will help guide activities and outline the planned mental health services, reflecting priorities as identified in the Needs Assessment, to be commissioned for the period from 1 July 2016 to 30 June 2017. 


Adelaide PHN Drug and Alcohol Treatment Activity Work Plan

The Drug and Alcohol Treatment Activity Work Plan will help guide activities and outline the planned Alcohol and Other Drugs (AOD) treatment services, reflecting priorities as identified in the Needs Assessment, to be commissioned for the period from 01 October 2016 to 30 June 2019. 

 

Adelaide PHN Integrated Team Care Funding Activity Work Plan

The Integrated Team Care Activity Work Plan outlines the Adelaide PHNs direction for commissioning of the Closing the Gap program from July 1 2016. Adelaide PHN will work with the provider to implement flexible approaches to best meet local community needs, with the aim to improve Aboriginal and Torres Strait Islander peoples access to culturally appropriate health care.


The key objectives of Primary Health Networks (PHN) are:

  • Increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and
  • Improving coordination of care to ensure patients receive the right care in the right place at the right time.
