The Adelaide PHN (APHN) receives funding from the Australian Government Department of Health to commission services which meet the health needs of the Adelaide metropolitan community.

The APHN Commissioning Governance Framework addresses the five phases of the Commissioning Cycle, click here to see the APHN Commissioning Cycle diagram.

The APHN has established an e-Procurement website through TenderLink, to provide health service providers, contractors, consultants and suppliers with access to a range of commissioning opportunities. Please register and create a profile with Tenderlink  to receive email alerts of new commissioning opportunities as they are advertised.

After Hours Primary
Mental Health
Services

To provide Adelaide metro-wide timely access to mental health assessment, support and navigation of services in the sociable after-hours time period in-order to reduce inappropriate attendances at Emergency Departments, be proactive in the de-escalation of mental health distress and provide support in the navigation and access to mainstream in-hours mental health services and improve the consumer experience.

 

 

Preventive Health
Care and Community Health Literacy

The activity aims to increase health literacy and knowledge of primary health care services system and preventative health choices available, to increase the capacity of primary health care providers to promote evidence-based preventative health activities and to strengthen the capacity of NGOs and LGAs to support consumers to make healthy lifestyle choices through the provision of collaborative approaches focussed on improving population health literacy and access to health information and services.

 

 

Hospital Avoidance
Program

This project aims to assess the feasibility and efficacy of coordinated, timely general practitioner visits, to reduce readmissions in older patients hospitalised with comorbidities. This project is part of the Adelaide PHN’s Care Connections initiative. 

 

 

Coordinated Cancer Screening for Aboriginal
and Torres Strait
Islander Women

This project will aim to improve awareness, access and participation in breast, bowel and cervical cancer screening for Aboriginal and Torres Strait Islander women, and comprise activities which assist with the implementation of recommendations from both the National Aboriginal and Torres Strait Islander Cancer Framework and the South Australian Aboriginal Cancer Control Plan regarding screening and early detection of cancer in Aboriginal and Torres Strait Islander people.

 

 

Palliative Care
Access to Medicines

This activity will aim to increase prescriber and community pharmacy knowledge and collaboration across metropolitan Adelaide, with a focus on a previously developed Core Medicines List (CML) to improve medication access and enable death at home.

 

 

After Hours Program

The aim of this program is to provide residents of aged care facilities with in - hours pro-active primary care to reduce hospital presentations in the after - hours period, to provide residents of aged care facilities with in-hours pro-active primary care to reduce GP attendance in the after-hours period and to build capacity within residential aged care to provide pro-active primary care in - hours to reduce hospital presentations and GP attendance in the after-hours period. A separate small grants program targeting General Practices and Pharmacies may be released. 

 

 

Northern Adelaide
General Practice
Liaison Unit 

The aim of this activity is to improve patient care by facilitating and strengthening health care collaboration, communication and integration between general practice and the Lyell McEwen Hospital in the Northern Adelaide Local Health Network.

 

 

Southern Adelaide
General Practice
Liaison Unit 

The aim of this activity is to improve patient care by facilitating and strengthening health care collaboration, communication and integration between general practice and the Flinders Medical Centre in the Southern Adelaide Local Health Network.
