General Practice Liaison Unit at the Women’s and Children’s Hospital

Working together for right care, right time, right place

Adelaide PHN, Country SA PHN and the Women’s and Children’s Health Network are pleased to announce the establishment of a General Practice Liaison Unit (GPLU) within the Paediatric Medicine Unit of the Women’s and Children’s Hospital in Adelaide.   

The GPLU will focus on improving the care of children and young people (aged 1 – 18 years of age) with chronic conditions such as asthma and type one diabetes who have frequent contact with the hospital by facilitating and strengthening collaboration, communication and integration with general practice. 

The GPLU will deliver programs of work across the following:

  • Engagement and Relationship Building
  • Communication and Collaboration
  • Access and Navigation
  • Capacity and Capability Building
  • Integration

The GPLU team includes a General Practitioner, registered nurse and administrative support officer commenced in January 2017.
