Community pharmacists play an important role in supporting families caring for a person with a life limiting illness who chooses to die at home.
To support pharmacists, caring@home have developed the Community Palliative Care Resources Pharmacy Pack.
The pack contains printed resources that pharmacists can give to families and carers who are caring for a person at home including providing practical care, physical symptom management and subcutaneous medicine management. This can help support quality and timely care for home-based patients.
Pharmacies are encouraged to:
- Order 1 pack per pharmacy
- Keep the pack as a master set and download/print more copies from the webpage as needed.
