About the Service
My Home Hospital (MyHH) is a SA Health service that provides eligible patients with hospital-level care in the comfort and privacy of their own homes, including aged care homes and disability supported accommodation – at no cost to the patient.

Comprehensive care
- Admission under credentialled medical officers, and daily medical ward rounding
- At least daily in-home care delivered by registered nurses and allied health practitioners (most patients have BD or TDS visits)
- Medication, pathology, diagnostics, oxygen, transport and other support services all included
Easy referral process
- Referrals accepted 24/7: Call 1800 111 644 for a clinician-to-clinician conversation
- Responsive, accessible team available for pre-referral discussion and support (including planned referrals to support acute post-operative admission)
Improved patient flow
- Supports early discharge, freeing up hospital beds and reducing inpatient demand.
- Support for elective and emergency surgery pathways, enabling same-day discharge (or reduced length of stay) with post-operative support at home
- Direct community admission pathways available, reducing pressure on emergency departments and inpatient wards
What are the clinical conditions treated?
MyHH manages a wide range of acute conditions, including but not limited to:
- Infections requiring IV antibiotics, like cellulitis, pneumonia and UTI
- Heart failure
- Respiratory conditions, like exacerbation of COPD
- Acute post-operative care
- Gastrointestinal, electrolyte, and coagulation disorders
And any other condition safe and appropriate for hospital-level care at home
MyHH is available to eligible public patients in metro Adelaide, Gawler and Mount Barker and surrounds, and selected areas of the Southern Fleurieu Peninsula. Call 1800 111 644 24/7 to refer or discuss a patient.
