About this resource

The MyHeart My Life program is for patients and their carers who have experienced a heart attack and/or angina admission in the last 12 months.
Patients receive two printed booklets in hospital during their acute admission. The first booklet ‘A guide to improving heart health’ and the second booklet: ‘Living well with heart disease’. These booklets are designed to help facilitate education about coronary heart disease and support ongoing secondary prevention.

Upon signing up to the MyHeart MyLife program, patients receive a supportive phone call from the iCCnet Country Access Cardiac Health team and a 6-month email journey guiding them through their initial recovery phase.

Encourage your patients to sign up to receive extra support;
