Adelaide PHN is excited to share the upcoming series of GPACI webinars.
Presented by Prestantia with these webinars will focus on implementing the General Practice in Aged Care Incentive, a new initiative from the Department of Health and Aged Care. This program supports GPs and healthcare providers in delivering consistent, proactive care for MyMedicare registered patients living in aged care homes.
Webinars will include conversations on:
- What this incentive means for you and the potential benefits
- How to utilise the incentive to support high quality, integrated, patient centred care in an aged care setting
- How obstacles involved in operationalising this incentive can be overcome.
