Adelaide PHNs Practice Health Check Survey

Adelaide PHN is the local Primary Health Network (PHN) for the Adelaide region. Our vision is to shape the health system to deliver better outcomes for vulnerable people, and a healthier Adelaide.  

Adelaide PHN commissions a range of services, programs, and activities to support and build capacity of the primary health care workforce, with a focus on general practice. 

To enable us to support general practice and practice teams we are looking to gain further insights into the general practices located within the Adelaide PHN region.  Your participation in this Practice Health Check Survey is incredibly valuable, will assist us to understand the composition of general practices in the region and identify and understand the areas in which practices and practice teams could benefit from support.  

Consent should be provided by an authorised person on behalf of your practice.
