At our recent state-wide Care Finder network meeting, Adelaide PHN and Country SA PHN had the privilege of welcoming the Exceptional Needs Unit (ENU) from the Department of Human Services, who delivered a presentation which illuminated the vital work in supporting individuals and families with multiple complex needs – those often facing significant facing barriers to accessing appropriate services.

Through a deep dive into the Exceptional Needs Response and Care Service Pathway, care finders gained clarity on complex support they provide to people under 65 years old, and to embed these pathways into their everyday practice.

While most care finder clients are 65 years or older, some people under 65 are eligible for care finder support, therefore the care finder's awareness of the ENU is so important.

The members shared case studies involving an older person experiencing homelessness, chronic health issues, and social isolation.

Previously, navigating support systems had been overwhelming and ineffective. With the new understanding of ENU pathways, the care finders were able to initiate a referral that led to a coordinated response.

The session was a turning point in strengthening collaboration between Care Finder and the ENU program.

About ENU
The ENU team works with systems and services to help support people and families who have multiple complex needs and risk factors. The ENU works with people and families who are facing barriers to accessing appropriate supports or who are struggling to get their complex needs well met by existing services. Learn more about ENU.

About Care Finder program
Care finders can help vulnerable older people navigate the aged care system and find support services to improve their quality of life. Care finders can help with both accessing services for the first time and changing or finding new services and supports.
