Co-designing an integrated care model for chronic conditions across northern Adelaide

Flinders University, Northern Adelaide Local Health Network (NALHN) and Adelaide PHN are seeking people living with diagnosed heart failure to be involved in interviews and codesign. 

They are looking for adults living with diagnosed heart failure to take part in:
 

  • a short, recorded interview. and/or-
  • a co-design workshop
     

They are keen to hear about your healthcare experiences. 

Sharing your experiences will help us to design a more accessible care model for people living with heart failure and other long-term health conditions in North Adelaide.

They are seeking individuals
who are:
 

  • 18 years-old and over
     
  • living with diagnosed heart failure
     
  • are using or have used healthcare
     
  • services in Northern Adelaide
     

What's involved in participating? 
 

A researcher will chat with you at a time and location of your choice, or online for a 30-6- minute interview. Receive a $50 gift card for participating in an interview and/or participate in a 2hr co-design workshop and receive a $35 gift card.
