Commissioning Now: Care Finder Program

Adelaide PHN is now pleased to invite applicants from suitably qualified and experienced organisations to deliver the Care Finder Program. 
This program aims to deliver specialist and intensive assistance to help people within the target population to understand and access aged care and connect with other relevant supports in the Adelaide PHN region. It’s aimed to specifically target senior Australians who have one or more reasons for requiring intensive support to interact with My Aged Care (either through the website, contact centre or face-to-face in Services Australia service centres), access aged care services and/or access other relevant supports in the community.
The services are expected to be delivered from 1 January 2023 to 30 June 2025.
A market briefing will be held via Zoom at 1.00pm on Thursday 29 September 2022 to provide an overview of the RFP and the process for submission. Further details can be found in the Tender documentation.
