Expression of Interest for MDT Care Project for Small and Solo General Practices

All accredited small and solo general practices within the Adelaide PHN region are invited to submit an Expression of Interest (EOI) for the Multidisciplinary Team (MDT) Care Project for Small and Solo General Practices.

The project aims to support small and solo general practices across the Adelaide PHN region with chronic condition clinical coordination support to assist in facilitating multidiscplinary team care. 

This includes funding to engage either a practice nurse or care coordinator to lead patient care coordination one to two days a week to support the patient and general practitioner to coordinate, communicate and integrate patient care with allied health providers.

The EOI is available on the TenderLink e-procurement portal. Interested small and solo general practices are advised to register on TenderLink to access and submit their EOI.
