
Flourish Primary Care Innovation Grants Open

Adelaide PHN will provide 10 grants of $10,000 each to primary care providers across our region to fund innovative solutions that tackle health disparities, enhance service access and empower underserved populations.
We're looking for projects that improve access to safe, timely and appropriate services for populations facing health inequities in the Adelaide region, including:
- Families, children, and young people
- People living with chronic or complex conditions
- Culturally diverse communities
- LGBTIQA+ individuals
- Aboriginal and Torres Strait Islander peoples
- People living with disabilities
- Communities experiencing socioeconomic disadvantage
Intended Outcomes:
Although the grant funding term is for nine months, it is hoped that the proposed solutions have a focus on developing sustainable solutions that will help shift access and equity towards the following longer-term outcomes, which are in line with Adelaide PHN's priority statements:
- Families, children and young people can access appropriate and timely early intervention, prevention and support.
- People at risk of developing or living with chronic or complex conditions can receive timely and appropriate interventions, care, support and management.
- Reduced preventable hospitalisations for people with chronic and complex conditions living in the Adelaide PHN region.
- Culturally and Linguistically Diverse communities (including newly arrived and refugee communities) can access timely, culturally safe and appropriate primary health care services.
- LGBTIQA+ communities can access timely, culturally safe and appropriate primary health care services.
- Aboriginal and Torres Strait Islander people can access culturally safe and appropriate workforce and primary care services, including services in a timely manner that support social and emotional wellbeing and spiritual healing.
- Aboriginal and Torres Strait Island people can access transgenerational trauma and grief counselling services and narrative therapies for suicide prevention and mental health.
- People in the Adelaide PHN have awareness of, and timely access to person-centred models of comprehensive care across the care continuum.
- People living with a disability can access safe, inclusive and appropriate health care services.
Project Timeline: Grant funding for these projects will run for nine (9) months from 1 July 2025 to 31 March 2026. Participating practices/providers will receive payments tied to specific project activities and milestones.
