Forecasting and Monitoring Tools - General Practice in Aged Care Incentive

Resources are now available to support forecasting payments and eligibility for the General Practice in Aged Care Incentive. Services Australia have resources and tools to support both practices and providers

Available Templates for Supporting GPACI Servicing Requirements: 

To further support practices and providers, the following customisable templates have been made available: 

• Care Plan Contribution Template: A standardised template for documenting care plan contributions, ensuring compliance with GPACI requirements and easy access for audits. 
• Patient Front Sheet Template: A quick reference tool to track a resident’s servicing needs over 12 months, including enrolment status and services provided, ensuring all requirements for GPACI are met. 
• Patient Monitoring and Tracking Tool: A tool to track patient care and completion of incentive-related activities, ensuring care plans are up to date and servicing requirements are met.
