Adelaide Primary Health Network (Adelaide PHN) has funded this program for 15 Residential Aged Care Home Organisations, with successful organisations receiving:

Two onsite change management workshops delivered by Thrive Aged Care Consultants

Practical tools and strategies to lead and sustain change

Support to implement a real service improvement initiative

Assistance to increase telehealth capability and adoption within your service

Expressions of Interest are now open and will close in three weeks.

With only 15 places available, we encourage interested services to submit their Expression of Interest as soon as possible.
