Aged care service providers play important role in the new End-of-Life pathway by initiating the referral process, and by supporting the planning and coordination of care.
Palliative Care Connect will deliver two informative webinars covering the new End-of-Life Pathway, part of the Support at Home program which commenced 1 November 2025 as part of the Aged Care Act 2024.
Tailored to aged care service providers, practice nurses, GPs and other primary health professionals, this webinar will provide a broad overview of the End-of-Life Pathway, which has been designed to provide enhanced support for older people to remain at home/ in place during end-of-life.
