General Practice in Aged Care Incentive (GPACI) Project

Adelaide PHN is seeking to commission four general practices to participate in the delivery of the General Practice in Aged Care Incentive (GPACI) Project.

The Project aims to enhance the experience, efficiency, and clinical effectiveness of GPs providing care within RACHs to support general practice teams to utilise GPACI effectively, increase workforce capability and support coordination of care between aged care residents and the general practitioner, ultimately leading to better resident outcomes and a more sustainable and satisfied general practice team.

Each participating general practice:

  • is required to nominate a RACH that they will be working with on the GPACI Project in their application.
  • will receive support from Adelaide PHN and an Adelaide PHN Commissioned Primary Healthcare Consultant (Bollen Health) during the term of the Project.
  • will receive incentive payment for their participation.

The successful applicants will be expected to participate in the project for a period of 10 months from the date of contract execution with the funding period running across 2025-2027 financial years.

A Project information session will be hosted by Adelaide PHN for the GPACI Project on 5 March 2026 at 12pm (ACDT). Interested applicants are required to register for the briefing HERE. The session will outline key project details and the application process.
