Heart Foundation’s brand new MyHeart MyLife resource hub
What is MyHeart MyLife?
MyHeart MyLife is the Heart Foundation’s free evidence-based support program for people living with coronary heart disease and their carers. The program aims to improve participants’ ability to self-manage their condition and empower them to live well with coronary heart disease.
The 12-week digital program:
- complements chronic disease management and cardiac rehabilitation programs, and bridges the gap between specialist and GP appointments
- provides patients and carers with evidence-based information and education presented via online articles, videos and animations
- offers tailored content based on participants’ recency of diagnosis and whether they live in a rural or remote location
- encourages improved adherence to medicines and healthy habits post-discharge from hospital
- supports emotional and psychosocial wellbeing and connection.
New MyHeart MyLife resource hub
To support primary healthcare professionals with patient education and referral to the program, we have launched a comprehensive suite of new print and downloadable resources:
- referral flyers for the primary care setting with information about the program and tips for living well with heart disease
- waiting room posters for general practices
- program guide for healthcare professionals to explore all articles, videos, tools and resources that patients have access to when they join MyHeart MyLife
- ready-to-print articles on key topics including blood pressure, cholesterol and heart-healthy eating
- printable discharge resource for patients newly diagnosed with coronary heart disease; particularly useful for those who are unable to access the digital program.
Plus, primary healthcare professionals can explore our other resources, including medicine management tools, tailored resources for patients living in rural and remote areas and warning signs of a heart attack action plans.
