Launching Adelaide PHN's 2026-2029 Strategic Plan
Adelaide PHN was proud to launch our 2026-2029 Strategic Plan at the 2026 Health Equity Awards.
This Strategic Plan sets a clear direction for how we will lead, partner and invest to improve health outcomes across our region. It reflects what we have heard through meaningful conversations and embeds our core goal - to support primary care, commission services where gaps exist and strengthen integration across the health system.
The launch of our new plan also provides an opportunity to reflect on our progress over the past three years of our 2023-2026 Strategic Plan.
With a focus on stakeholder engagement, place-based commissioning and strong partnerships, we have contributed to population health outcomes, enhanced consumer experience and more coordinated, person-centred care.
Over the past three years, we have strengthened community engagement through Kitchen Table discussions and Yarning Circles, while also building on our partnerships with Local Health Networks (LHNs), Aboriginal Community Controlled Organisations and researchers, including our ongoing work alongside Flinders University and its Caring Futures Institute.
We introduced our Access and Eqity Framework, while also strengthening cultural capability across our organisation, launching our 2026-2028 Innovate Reconciliation Action Plan earlier this year.
Other key achievements include improved access to aged care and homelessness services, stronger support for refugee communities and expanded mental health services such as the Northern Adelaide Medicare Mental Health Centre.
Our new Strategic Plan moves from building foundations to increasing our impact. As Adelaide PHN's Board Chair, Dr Melanie Smith reflected at the awards night, "It reinforces our commitment to delivering integrated, high-value care that improves health outcomes, reduces inequities and strengthens a more coordinated and sustainable health system for our community."
