Local Link Service for Supporting the Primary Care Response to Family, Domestic and Sexual Violence pilot project
Adelaide PHN has a funded opportunity for private not-for-profit and private for-profit organisations with a substantial local presence and demonstrated experience in delivering Family, Domestic and Sexual Violence (FDSV) related services to participate in the Supporting the Primary Care Response to Family, Domestic and Sexual Violence pilot project.
The pilot project aims to create meaningful change for people experiencing or at risk of experiencing FDSV including CSA by providing strategies for the primary care workforce to Recognise, Respond to, and Refer patients experiencing FDSV, including CSA.
Overview
The successful applicant will be required to deliver high-quality, responsive, and timely services as the FDSV Pilot Project: Local Link Provider within the northern metropolitan area of Adelaide, specifically encompassing the City of Playford and the City of Salisbury and surrounds.
This will include delivering timely support to the primary care workforce and patients concerning FDSV and CSA response needs.
The successful applicant will implement a local link team to deliver the following core functions:
- Primary Care Support: Provide practical guidance and implementation support to help primary care providers develop trauma-responsive approaches
- Patient Support: Deliver holistic case management and system navigation support, regardless of how patients enter the system
- System/Local Integration: Work to improve how services operate together
Project timeline: The project initiative will run through until 30 June 2027
