New Embedded Referral Form for Adelaide Universal Aftercare Service

Referring to the Adelaide Universal Aftercare Service is now easier.

The Universal Aftercare Program Referral Form has been converted to an RTF format that can be embedded into Best Practice and Zedmed clinical software, allowing clinicians to complete referrals more efficiently within their existing workflow.

Delivered by AnglicareSA, the Adelaide Universal Aftercare Service is a free outreach program providing non-clinical psychosocial and practical support for people experiencing suicidal thoughts or distress related to psychosocial stressors across metropolitan Adelaide (NALHN, CALHN and SALHN regions).

Referrals are reviewed within 24 hours of receipt, with initial contact and support generally provided within 7 days.
