RACHs Change Management Training – Express your interest now!

Adelaide PHN is inviting Residential Aged Care Home sites the opportunity to take part in a free Change Management Training Program delivered onsite at your location!  

Adelaide Primary Health Network (Adelaide PHN) has funding for 15 Residential Aged Care Home sites with successful sites receiving: 

• Two onsite change management workshops delivered by Thrive Aged Care Consultants 
• Practical tools and strategies to lead and sustain change 
• Support to implement a real service improvement initiative 
• Assistance to increase telehealth capability and adoption within your service

This program is delivered by Thrive Aged Care Consultants and provides practical, hands-on support to help your team embed digital health into everyday practice. 

Expressions of Interest

The Expression of Interest (EOI) period has been extended and EOIs are now site-based, giving individual RACHs flexibility to nominate their own team.
