Recruitment of General Practices for Northern Adelaide Wraparound Care Pilot Project

Adelaide PHN is excited to announce the Northern Adelaide Wraparound Care Pilot Project aimed at supporting patients with complex chronic conditions through comprehensive multidisciplinary team (MDT) care. This pilot will leverage digital technology and allied health providers to enhance care coordination, reduce the burden on the tertiary health system, and strengthen the infrastructure and resources among health professionals in the Northern Adelaide region. 

Project Overview: The Northern Adelaide Wraparound Care Pilot seeks to: 

• Identify and support access for patients with complex chronic health conditions who would benefit from MDT care.
• Increase patient and community awareness of allied health providers.
• Address underlying health issues, including social determinants of health and barriers to accessing healthcare.
• Build on evidence-based models of integrating allied health professionals into MDT care. 

Intended Outcomes: 

• Improved patient outcomes through coordinated MDT care.
• Enhanced understanding and utilisation of MDT care among general practices.
• Increased awareness and collaboration among allied health providers.
• Reduction in potentially preventable hospital admissions and emergency department presentations.
• Decreased healthcare costs through coordinated services and avoiding unnecessary hospital admissions 

Project Timeline: The pilot will commence on 29 May 2025 and continue for 24 months. Participating general practices will receive payments tied to specific project activities and milestones.  

This project further supports to upgrade digital health capabilities of the GP practices and allied health professionals involved in the project.

Invitation to Market Briefing: General practices in the Northern Adelaide Local Health Network (NALHN) region are invited to attend a virtual market briefing on 15 April 2025 at 12:30 pm ACDT. During this briefing, detailed project information will be provided. 

Practices interested in attending the briefing are requested to send an email to  applications@adelaidephn.com.au to register their interest and receive access details for the briefing.
