Request for Tender for Early Intervention Project – Phase 3
All general practices within the Central Adelaide Local Health Network Area are invited to submit a Request for Tender (RFT) for the Early Intervention Initiatives to Support Healthy Ageing and Ongoing Management of Chronic Conditions Phase 3 Project (Early Intervention Phase Project).
The Project seeks to support older Australians living in the Adelaide PHN to live well in the community through enhancing understanding, awareness and skills of the local primary health care workforce. The Early Intervention Phase 3 Project also aims to maximise the implementation of evidence informed early intervention initiatives promoting healthy ageing to foster sustainability.
Practices will receive:
Support from Adelaide PHN, Flinders University Caring Futures Institute and the Healthcare Consultant during the term of the Project
Structured mentoring, strong leadership and peer learning on social prescribing, advanced care planning and undertaking health assessments.
Support to identify local community groups that will assist with social prescribing and referral pathways.
Embedding healthcare screening into routine practice
Integration of screening tools into existing workflows with the aim of reducing hospitalisations
Improve multidisciplinary collaboration and early intervention
Multiple modes of commissioned activity used during phase 1 and 2 of Early Intervention projects has resulted in an increase in workforce capability for general practice, uptake of QI tools and improved quality of life for older people in the Adelaide PHN region. Adelaide PHN aims to continue to promote healthy aging by utilising health assessments including advanced care planning, social prescribing and increasing nursing capability as identified QI activities.
In addition, it is anticipated that the Project will enable the continuous support of general practice training and development, quality improvement toolkits and capacity building activities for health professionals across the Adelaide PHN region via ECHOs. These would help to strengthen the role of the primary care sector in reducing the burden on the tertiary health system and provide equitable access to early intervention services when and where it is needed.
The Project services are to be delivered for a period of 18 months across 2025 – 2027 financial years.
