Strengthening Pathways for Elders: Adelaide PHN Contributes to State Yarning Circle

Adelaide PHN recently attended the Elder Care Support State Yarning Circle in Mount Gambier, home to the Boandik Peoples.

The Elder Care Support (ECS) program supports Elders and older Aboriginal and Torres Strait Islander people to understand the aged care system, including identifying the level of care and services required.

The three-day event brought together key stakeholders from across the state, including Aboriginal organisations that run the ECS program, Department of Health and Aged Care, Aged Rights Advocacy Service (ARAS), Country SA PHN, Dementia Australia, Services Australia, and more.

Our Care Finder Contracts and Capacity Building Coordinator Lashya Krishnamurthy, alongside Country SA PHN Care Finder program lead Helen Morley, had the opportunity to present on the second national care finder evaluation, including key finders and lessons learned, an overview of the national data and how this links with the continuous improvement of the program.

Engagement with Elder Care Support Coordinators and Connectors offered valuable insights into regional needs and reaffirmed our commitment to supporting older community members within the Adelaide PHN region.

The yarning circle initiated discussions to support each other's referral pathways and program queries. There has been a strong focus on ensuring PHNs and ECS organisations engage and explore ways in which we can strengthen the program that we are supporting in our regions and assist any potential shared clients.
