Support at Home Program End-of-Life Pathway Information Webinars

Country SA PHN, Adelaide PHN, and Palliative Care Connect have partnered to deliver two informative webinars covering the new End-of-Life Pathway, part of the Support at Home program which commenced 1 November 2025 as part of the Aged Care Act 2024. 
These online webinars are tailored for GPs, practice nurses, and other primary health and aged care professionals, and will provide a broad overview of the End-of-Life Pathway, which has been designed to provide enhanced support for older people to remain at home/ in place during end-of-life.

There will be two 45 minute webinar sessions held on Wednesday 3 December 2025:
Session 1: 1:00 PM to 1:45 PM Register now
Session 2: 6:00 PM to 6:45 PM Register now

Please note that both sessions cover the same content. 
Topics covered
•    New provisions, eligibility, access, and funding
•    Prognostic criteria and Australian-modified Karnofsky Performance Scale (AKPS) scoring
•    Referral options and collaboration with palliative care services
•    Common questions and where to find further information
Presenters
Palliative Care Connect Navigators will provide an overview of the End-of-Life Pathway and how it will impact care. They will also highlight the role of Palliative Care Connect in supporting patients and their loved ones through end-of-life planning and coordination.
