Support Your Patients Living with Persistent Pain

Persistent pain can significantly impact a patient’s quality of life. When pain lasts longer than three months, traditional approaches often fall short. The Living Well with Persistent Pain (LWwPP) Program offers a free, evidence-based solution to help patients manage pain effectively and improve daily functioning.

Why Refer Your Patients?

The LWwPP Program focuses on education and self-management strategies rather than seeking a cure. Patients receive:

  • Group education sessions on understanding pain and approaches that support recovery.
  • Individual consultations to tailor strategies to patient needs.
  • Access to allied health professionals, including physiotherapists, dietitians, pharmacists, mental health clinicians, and exercise physiologists.
  • Care coordination to support collaboration between the patient, their GP, and the program team.

Eligibility Criteria

Patients must:

  • Be over 18 years old.
  • Have persistent pain for more than 3–6 months.
  • Be suitable for group education and able to understand English.
  • Be willing to provide consent for de-identified data collection.

Please note: The program does not provide surgical or specialist pain services and does not affect hospital pain clinic waiting lists.

How to Refer

Eligible patients must be referred by a GP or specialist. Allied health providers can initiate referrals, but GP or specialist sign-off is required.
