Survey- Onsite Pharmacist Implementation Trial 

Invitation to the Survey- Onsite Pharmacist Implementation Trial in Residential Aged Care Homes in Urban, Regional, Rural, and Remote Australia (OPTIMISER3) 

We are currently gathering feedback to better understand the implementation of the Aged Care Onsite Pharmacist (ACOP) program, and your perspective would be greatly valuable. 

The survey is designed for professionals involved in or impacted by the ACOP model, including pharmacists, ACOPs, aged care managers, Primary Health Networks, general practitioners GPs, and nursing staff. 

The survey will take approximately 10 minutes to complete, and all responses will remain anonymous and confidential.
