Adelaide PHN is proud to support the Australian Government’s Aged Care On-Site Pharmacist (ACOP) Measure, a key initiative to improve medication safety and quality of care in Residential Aged Care Homes (RACHs).
Developed by the Department of Health, Aged Care and Disability (DHAD), ACOP responds to recommendations from the Royal Commission into Aged Care Quality and Safety. On-site pharmacists provide expert medication support, improve prescribing practices, and strengthen collaboration across care teams.

Thanks to all who attended the webinar titled ‘An Overview of the Aged Care On-site Pharmacist (ACOP) Measure’, held on 28 August 2025. We had a tremendous turnout with a diverse range of participants from Residential Aged Care Homes, community pharmacies, and pharmacists.
The ACOP Measure has been in operation since 1 July 2024. Uptake continues to grow, with around 15% of residential aged care homes (RACHs) in South Australia now taking up the opportunity. Participation is similar across large and small aged care homes. Read the full report here.
