The Medicare Mental Health SmartForm is now available in HealthLink

Adelaide metropolitan GPs and other health professionals using HealthLink-enabled clinical software can now refer patients experiencing mental health challenges directly to the Medicare Mental Health Phone Service using the HealthLink SmartForm from within the patient’s record.

Why? 

E-referrals bring significant benefits for referrers and consumers including:

  • No faxing or printing
  • Faster processing = faster access for patients
  • Instant confirmation that referrals have been received
  • All information provided is securely stored
  • Progress of the referral can be tracked throughout the process

How?

  1. Open the patient record
  2. Navigate to HealthLink SmartForms in your clinical software
  3. Select Medicare Mental Health (1800 595 212) from the SmartForms service list
  4. Choose your state and PHN region
  5. Complete and submit the form
  6. Check your results inbox for status updates
