Session One: Deterioration and Decision-Making After Hours
Topics include:

• Recognising and responding to deterioration

Guest Speaker:
Dr Linda Appiah-Kubi is a Consultant Geriatrician at Western Health with more than 20 years' experience in geriatric medicine across Australia and the UK. Passionate about improving care for older people, Linda is a clinical leader, educator and mentor who works closely with healthcare teams to support high-quality, person-centred care. She also has extensive experience in clinical training, communication, and managing complex care needs for older adults.

• Andrea Russell, Director of Clinical Services at Great Ocean Road Health
  As a Registered Nurse with more than 30 years of experience in aged care and healthcare across Australia and the UK. Passionate about improving outcomes for older Australians, Andrea has seen the aged care sector evolve significantly throughout her career and continues to champion person-centred, high-quality care. Known for her practical approach, she brings real-world stories, proven strategies and achievable tips to help residential aged care homes improve care outcomes—regardless of their size, circumstances or current quality indicator results. She is also a PhD candidate at Deakin University researching consumer choice and risk enablement in falls prevention in residential aged care..
• Peter Williams, CEO at One Care Limited Tasmania
  As the CEO of One Care Limited, one of Tasmania's leading not for profit aged care providers. beginning his healthcare career as a Personal Care Assistant in 1990, he qualifies as a Registered Nurse in 1993, with postgraduate qualifications in clinical education and business. A Tasmanian Churchill Fellow, Pete was awardee the national Meg Gilmartin Scholarship in 20222 for his research into international models of ageing and reablement.

He contributes to sector reform through several leadership roles, including as Chair of Ageing Australia's Tasmanian State Member Council, member of the Older Tasmanians Ministerial Advisory Council, and member of the National Preventative Mechanism Council. Pete is committed to advancing quality, dignity and innovation in aged care, with a particular interest in wellbeing, health literacy and reshaping societal narratives around ageing.

Session Three: Making Every Record Count: Using Health Records to Improve Care the meet the New Standards

• What good documentation looks like under the strengthened Standards
• My Health Record in practice: supporting better communication across care teams
• Using health information to support clinical decisions and reduce risk
• Common challenges in health record management and practical solutions
• Preparing your service for increasingly connected models of car

Guest Speaker:
InnovAgeing - The InnovAgeing team brings extensive experience supporting residential aged care homes to strengthen digital maturity and adopt digital health solutions that improve care quality and clinical decision-making. In this session, they will share practical insights into the role of health records, including My Health Record, in supporting continuity of care, information sharing, and better resident outcomes. Attendees will gain practical strategies to strengthen documentation practices, leverage digital health tools effectively, and ensure health records not only support compliance with the new Standards but also contribute to safer, more connected and person-centred care.
