Early Interventions Initiatives to Support Healthy Ageing and Ongoing Management of Chronic Conditions – Project ECHO

Our Project ECHO® is a series of interactive virtual learning sessions delivered via Zoom that support GPs and health professionals across the Adelaide PHN region to build knowledge, skills, and confidence in promoting healthy ageing and optimising the management of chronic conditions in primary care settings.

This free five-part series focuses on practical early intervention approaches that help identify risk factors, recognise early signs of decline, support proactive care planning, and improve outcomes for older people and those living with chronic conditions. The series forms the first phase of a broader ten-session Project ECHO® program, with a further five-part series planned for 2027.

 

Each 75-minute Project ECHO® session provides a safe, supportive, and collaborative learning environment and includes:

• A short presentation from an experienced subject matter expert.
• A de-identified case presented by a participant, allowing for discussion, specialist input, and shared problem-solving.
• Opportunities for questions, discussion, and peer-to-peer learning.
• Practical strategies and resources that can be applied in everyday clinical practice.

 

Participants will explore topics including:

• Healthy Ageing in General Practice: Identifying Early Decline
• Comprehensive Health Assessments: 75+ and Aboriginal and Torres Strait Islander 55+ Health Assessments
• Advance Care Planning in General Practice
• Chronic Disease Optimisation in Older Adults: Multimorbidity Focus
• Social Prescribing: Connecting Patients to Community Supports

 

There is no cost to attend, and benefits include:

Submit a Case

Case discussion is central to the Project ECHO® model. Participants are encouraged to submit de-identified cases that highlight common clinical presentations, early indicators of decline, multimorbidity, care planning challenges, social prescribing opportunities, or other issues encountered in everyday practice.

Sharing cases supports collective learning, clinical reflection, and the ongoing improvement of patient care.

We welcome GPs and health professionals across the Adelaide PHN region to join this free program and become part of a collaborative learning network dedicated to supporting healthy ageing and improving chronic disease outcomes.

 

You only need to register once to attend all five sessions in the 2026 series.

These sessions are RACGP-approved CPD activities under the RACGP CPD Program.

GPEx is an RACGP Accredited CPD Provider.

CPD hours and certificates will be issued following completion of the 2026 Project ECHO series in December 2026.
