Funded Opportunity: Quality Improvement activity in General Practice: Focusing on Frailty

Adelaide PHN has a funded opportunity for General Practices to implement a Quality Improvement activity with a Frailty focus on patients aged 65 and over. This Quality Improvement (QI) activity will be supported by a QI consultant commissioned by Adelaide PHN.  

The process for the application will be by an online application submitted on Tender link. 

This activity will benefit your practice with: 

• Better health outcomes for the ageing population. 
• Expert GP and Primary Care Nurse consultants to support your practice to undertake 90 days of QI activity. 
• Dedicated opportunity to encourage practice team activity and focus. 
• 3 x Community of Practices which translate into self-recorded CPD for all clinicians. 
• Tools to lead ongoing bursts of quality improvement activity within your practice independently. 
• Meeting the PIP-QI criteria. 
• Enhancing relationships with Allied Health Practitioners.
