This webinar will provide Healthcare Professionals with knowledge of the new pilot program called Palliative Care connect and its navigation component, the Statewide Palliative Care Navigation Service. 

Palliative Care Connect

Palliative Care Connect is a service funded by the Commonwealth Government, implemented by the South Australian Government. It aims to provide free support to all South Australians impacted by a life limiting illness, their families, carers, and the health professionals who care for them.

Its primary goal is to connect individuals with information, resources, and links to palliative and end of life services, empowering individuals to make choices aligned with their culture and preferences. It also offers bereavement supports to those navigating the challenging journey of loss.

Palliative care is a topic that touches us all, at some point in our lives.  Having open conversations can help dispel myths, reduce fear, provide much-needed support and along with the Primary Health Network, become part of the team supporting your patients.

What’s being presented?

  • An overview of the Palliative Care Connect pilot project and the various services and supports under the project.
  • The role of the Statewide palliative care Navigation service.
  • A case example outlining how Navigators might assist Healthcare Professionals and their patients.
