MDT Care Project for Small and Solo General Practices

All Small and Solo General Practices within the Adelaide PHN region are invited to a virtual Project information session, which will discuss all the key details associated with the Multi-disciplinary Team (MDT) Care Project, including the expectations around the Project delivery and the budget.

The MDT Care Project aims to support small and solo general practices across the Adelaide PHN region to engage clinical coordination support to facilitate multi-disciplinary team care. 

This includes funding to engage either a practice nurse or care coordinator to lead patient care coordination one to two days a week to support the patient and general practitioner to coordinate, communicate and integrate patient care with allied health providers.
