NiPHC Chronic Disease Management and Healthy Ageing Program

This virtual workshop is the first stage in our 6-month intermediate level Chronic Disease Management and Healthy Ageing Program. 

Program objectives

• To enhance understanding of the principles of chronic disease management and healthy ageing for five key chronic conditions.
• To incorporate and apply the concepts of chronic disease management and healthy ageing within nursing practice.
• To identify strengths and areas to grow in relation to chronic disease management and healthy ageing practice.

3. New South Wales: 4-6pm
4. Queensland: 4-6pm
5. South Australia: 3:30-5:30pm
6. Northern territory: 3:30-5:30pm
7. Western Australia (AWST): 2-4pm
