Geriatric Outreach Service (RACH) - Supporting older people who have dementia/progressive cognitive decline with changed behaviours
Residential aged care providers are invited by SA Health to attend an online forum regarding the SA Health Geriatric Outreach Services (GOS).
SA Health GOS supports older people who have dementia/ progressive cognitive decline with changed behaviours to:
- transition from hospital to residential aged care; working together with the older person, their family, treating medical team and aged care home, to prepare for and support the transition from hospital into the aged care home, and for a period post transition.
- prevent avoidable hospital admissions from residential aged care; working with aged care homes to support the older person to remain in place with in-reach support from hospital teams.
