Transforming Care: Launch of Health Pathways in Adult Trans Health and Gender-Affirming Hormone Therapy

ASHM and Brisbane South PHN are proud to collaborate on the launch of the following Health Pathways:

• Gender-affirming hormones
• Transgender Health and Gender Diversity in Adults

The Pathways are designed to support General Practitioners in providing inclusive, evidence-based care for trans and gender diverse patients aged 18 and over.

This GP-focused session will cover both the initiation and ongoing management of GAHT, equipping clinicians with the knowledge and confidence to support their patients effectively. The session will include case-based discussions to explore real-world scenarios, clinical considerations, and best practices in gender affirming care.

Learning outcomes:

• Assess patient suitability, initiate hormone therapy, and provide ongoing care following best practice guidelines
• Enhance decision-making skills by exploring real-world scenarios and addressing common challenges in gender affirming care
• Gain insights from healthcare and community experts and identify key resources to support patients on GAHT
