Health Equity Awards

Adelaide PHN hosts Health Equity Awards (previously Primary Health Care Awards) every year to recognise the hard work and dedication of our region’s primary health care organisations, who have been responsive to our community’s needs in an ever-changing environment. Winners are awarded and celebrated at an annual dinner.

Health Equity Awards 2026

We proudly celebrated our 2026 Health Equity Awards on 22 August at the Hotel Grand Chancellor Adelaide, an evening dedicated to recognising the outstanding primary healthcare providers across our region who are driving positive change and building a healthier and more equitable healthcare system.

The event brought together leaders from across the health sector, government and community, including Member for Boothby Louise Miller-Frost MP and South Australian Minister for Health and Wellbeing Blair Boyer, who shared reflections on the importance of advancing health equity.

The evening opened with a delightful musical performance by guitarist and vocalist Katie Aspel. Returning as our MC was award-winning journalist, producer and presenter Caroline Winter, while Robert Taylor was also back to deliver a moving Welcome to Country, reminding us of the profound connection between health, culture and community.

Award Recipients

This year, we were thrilled to recognise six organisations for their innovative and impactful approaches to improving health equity in their communities.

Their work is improving access to mental health services, supporting families, strengthening chronic disease care, enhancing eating disorder treatment pathways, reaching people experiencing homelessness, and delivering culturally responsive care.

Each initiative is creating meaningful change for individuals and communities facing barriers to accessing healthcare.

To showcase each winner, Mackbel Films produced six engaging videos that were featured on the night.

The winners also received a specially designed trophy created by Josh Rankine, owner of EtchTech Designs and a proud Ngarrindjeri man living on Kaurna Country. The design incorporates elements of our own Reconciliation artwork, featuring the central meeting place that represents Adelaide PHN.

The recipients of the 2026 Health Equity Awards are showcased below:

Skylight Mental Health’s Lived Experience Telephone Support Service (LETSS) provides free, after-hours mental health support delivered by people with lived experience of mental health challenges.

Operating every evening, the service helps thousands of people each year access safe, anonymous and non-judgemental support when they need it most.

Mission Australia's Integrated Family Case Work Model addresses a critical gap in alcohol and other drug services by supporting the entire family and support network, rather than focusing solely on the individual experiencing substance use challenges.

This family-inclusive approach has improved engagement, strengthened family wellbeing and helped create better outcomes for clients and their loved ones.

Supported through Adelaide PHN's Flourish Primary Care Innovation Grant, Pasadena Medical was recognised for Improving Chronic Condition Management and Health Equity through Community Engagement.

This initiative is improving access to chronic disease management for culturally diverse communities, Aboriginal and Torres Strait Islander peoples and those experiencing socioeconomic disadvantage. Through tailored care planning and community engagement, the project empowered patients to better understand and manage their health while increasing uptake of essential healthcare services.

The Right Care, Right Place project is strengthening the system's ability to identify and respond to eating disorders earlier and more effectively.

By building clinician confidence, improving referral pathways and supporting access to appropriate care, the project is helping more people receive timely, safe and coordinated treatment.

Uniting Communities' Streetlink Plus Primary Health Clinic delivers accessible primary healthcare to people experiencing homelessness in the Port Adelaide Enfield region.

By bringing care directly to those who face significant barriers to accessing traditional health services, the clinic is improving health outcomes and fostering ongoing engagement with preventative care.

The ACT Centre's Aboriginal Mental Health Treatment Program removes barriers to mental health for Aboriginal and Torres Strait Islander peoples by proving culturally safe, flexible and accessible support.

Through a strong focus on cultural responsiveness and person-centred care, the program is improving mental health outcomes while strengthening trust in healthcare services.
