Adelaide Integrated Respiratory Response (AIRR) Project - improving outcomes for people living with COPD and asthma
Building on the findings of the Adelaide Respiratory Health Project (ARHP), Adelaide PHN has commissioned Asthma Australia to deliver the AIRR: an initiative focused on promoting evidence based, person centered respiratory primary health care. The project facilitates collaboration between general practices & pharmacies & works to improve pathways between primary health care & Local Health Networks (LHNs). The AIRR engages patients with their care coordinators to build capacity & confidence in managing their own respiratory health & supports accredited training & resources for providers.
Referral Information
Asthma Australia is inviting expressions of interest from general practices and pharmacies that wish to be part of this innovative project, who together can improve patient outcomes for asthma and COPD.
