Living Well with Persistent Pain Program


When pain lasts longer than three months things need to change. A multi-disciplinary, evidence-based and collaborative primary care-based pain management program combining group education, individual care coordination and a tailored plan of allied health therapy. The program supports individuals to better understand their chronic pain, and develop the skills to better manage their pain, and improve their quality of life. The program aims to provide an accessible local service and is available for people who are currently waiting for hospital-based pain management services.

Referral Information

Living Well With Persistent Pain is a Painwise Program, © Painwise Pty Ltd.

Eligible consumers must be referred to the program by their GP or specialist. Whilst referrals can be initiated by allied health care providers, GP or specialist sign off must be obtained. 
Consumers are eligible if they are: 18 years of age or older, experiencing persistent pain which has lasted for more than 3-6 months, diagnosed with persistent pain, not suited for surgical or urgent pain specialist intervention, suitable for and willing to participate in group education, able to understand written and spoken English, willing to give consent for their de-identified data to be collected to evaluate the program.
