Central Adelaide General Practice Integration Unit (CAGPIU)


This unit is a joint project with Central Adelaide LHN to improve health outcomes for patients moving between primary and tertiary care. This program aims to reduce the number of avoidable hospital presentations and inappropriate referrals to the outpatient department at the Royal Adelaide Hospital and Queen Elizabeth Hospital from general practice, support improved communication, collaboration and integration between general practice and the hospitals, and improve health outcomes and experience for patients identified in the project cohort.

Clinical Suicide Prevention Services - immediate short term clinical intervention


Delivered by mental health clinicians at Sonder, Links To Wellbeing & Psychmed; clinical suicide prevention services provide immediate short term clinical intervention for people at risk of suicide or self-harm. Access to a mental health clinician is provided within 24hrs with in person appointments within 72hrs (excl. weekends & pub. hols). The services focus on risk assessment, management, skills development & short solution focused therapy. Clients at acute & immediate risk of suicide, self-harm, or harm to others will be appropriately referred to relevant emergency or crisis services.

Adelaide PHN’s Central Referral Unit (CRU) and Mental Health Enquiry Line ceased operation on 1 July 2023 and amalgamated with the Head to Health Phone Service.

Community Transitions Drug & Alcohol Programs - OARS


A program supporting those who are 18 years or older that have been recently released from the custody of the Department for Correctional Services, or in contact with the criminal justice system. The program offers individual counselling, drug awareness, relapse prevention and Self-Management and Recovery Training (SMART). Delivered by OARS (Offenders Aid Rehabilitation Services) Community Transitions, this program is one of a suite of AOD Treatment Services commissioned by Adelaide PHN.

Community services, health professionals and individuals may self-refer.
 
Services are available at three locations: Adelaide, Port Adelaide and Christies Beach.

Drug and Alcohol Services for People from Culturally and Linguistically Diverse (CALD) Communities


Free, voluntary, and culturally appropriate AOD treatment interventions for people aged 18 years or older from CALD communities living in the Adelaide metropolitan region. Treatment interventions include screening and assessments, brief interventions, case management, counselling, and support. Delivered by Community Access and Services SA (CAaSSA), this program is one of a suite of AOD treatment services commissioned by Adelaide PHN.

Referrals can be made by contacting the CAaSSA office via email or telephone or completing the CAaSSA referral form. Includes:

Drug and Alcohol Youth Outreach Service


The Drug and Alcohol Youth Outreach Service provides treatment intervention young people aged 10-25 years with mild and / or mild to moderate substance use issues. The service offers assertive outreach, counselling, care coordination, information and education sessions, peer support including peer support groups. Harm Minimisation Team - AOD Treatment Interventions for Aboriginal and Torres Strait Islander People


This program offers culturally appropriate treatment interventions to Aboriginal and Torres Strait Islander people aged 15 years and over with complex substance use issues. It also provides support for families and significant others who are impacted by a person’s problematic AOD use. Services include information and education; brief interventions; assessment; family support; case management (including care coordination, relapse prevention); counselling; and peer support. This program is one of a suite of AOD treatment services commissioned by Adelaide PHN.

Head to Health Phone Service and 24/7 Mental Health Clinic


Head to Health helps all Australians access the mental health and wellbeing services that are right for them. It makes it easier for people to navigate and choose the most suitable care options, whether that’s face-to-face, via phone or online. The Head to Health website is a gateway to digital mental health resources from trusted service providers. It helps people find and navigate the range of free and low-cost phone and online resources and services available, when and where they need it – in a private, secure and anonymous way. There are a range of digital resources and information on supports available at www.headtohealth.gov.au, and the Head to Health Phone Service 1800 595 212 (available 8.30am to 5.00pm weekdays) provides an accessible entry point to mental health advice, assessment, and referral to a broad range of mental health and wellbeing supports according to individual needs.

This may include referral into a Head to Health mental health centre, low intensity digital supports, mental health and wellbeing services commissioned by Primary Health Networks, mental health services funded by states and territories, or private services.

Head to Health centres are designed to provide:

  • Assistance to people experiencing a crisis or in significant distress including those at heightened risk of suicide — this may reduce presentations to emergency departments.
  • A central point to connect people to other services in the region.
  • In-house assessment, including information and support to access services.
  • Holistic, evidence-based care, in the immediate and short to medium term.

Head to Health centres are open to Australians who are seeking support for themselves, a friend, or family member without needing a referral, appointment, or any out-of-pocket costs.

headspace Centres - primary mental health services for young people


headspace Centres provide free early intervention focussed mental health services for young people aged 12-25 years. Adelaide PHN has commissioned centres across the Adelaide region in Edinburgh North, Onkaparinga, Adelaide, and Port Adelaide. These services are delivered by multiple providers and make up part of a suite of Adelaide PHN’s commissioned and specialised primary mental healthcare services.

Young people can access headspace centres directly by calling or visiting a headspace centre.

Referral forms can also be accessed by contacting the service providers or visiting their websites. 

A GP Mental Health Treatment plan is initially not required but can be helpful.

headspace Early Psychosis Program (previously known as hYEPP (headspace Youth Early Psychosis Program))


The headspace Early Psychosis Program provides early intervention, responsive, and recovery focused care for young people who are at high risk of or are experiencing a first episode of psychosis. This program follows the Early Psychosis Prevention and Intervention Centre (EPPIC) model of care. hYEPP is delivered by Sonder across the Adelaide region as part of a suite of specialised primary mental health services commissioned by Adelaide PHN.

Young people can access hYEPP services directly by calling or visiting the Adelaide centre.

Referral forms can also be accessed by contacting the service provider or visiting the website. 

A GP Mental Health Treatment plan is not required but can be helpful.

Thorne Harbour Health will provide a person-centred, evidence-based, high quality and culturally safe alcohol and other drug treatment service to lesbian, gay, bisexual, trans and gender diverse, intersex and queer (LGBTIQ) people residing in the Adelaide PHN region.

MATRIX Metamphetamine Treatment Program


Evidenced based, abstinence-oriented community treatment program for significant substance users aged 18+ undergoing drug replacement therapy with a medical practitioner. This may include methamphetamine or cocaine use or combined with opioid use. Run by trained psychologists and co-facilitated by workers with lived experience the program requires intensive commitment from people pursuing intermediate and long-term recovery. Delivered by PsychMed, it is one of a suite of AOD Treatment Services commissioned by Adelaide PHN.

MI-DBT - a specialised mental health program for women with BPD in the perinatal period


MI-DBT is a group therapy program for women living with Borderline Personality Disorder (BPD) in the Perinatal period. MI-DBT is an adaptation of Dialectical Behaviour Therapy (DBT) that aims to improve a woman’s emotional regulation, interpersonal effectiveness and parenting skills at a critical time in an infant’s development (within the first 1001 days). Sonder and Neami are commissioned to deliver these groups in the North and South of Adelaide respectively, with collaboration from the Helen Mayo House and the Department of Education and Child Services (DECS).

Multicultural Wellbeing Program - A Holistic Approach to Chronic Pain Management


Multicultural Wellbeing Program will develop local approaches to supporting people from culturally and linguistically diverse communities to self-manage and live well with persistent pain. The program includes gentle exercise and information sessions designed around each culture’s own understanding of pain and how to manage it. These sessions will end with the creation of an online video made by the participants, in their own language to help other members of their community to understand and manage pain.

Northern Adelaide Children and Family Mental Health Service - Lives Lived Well


Lives Lived Well offer free mental health services to families and their children 0-11 years in Northern Adelaide. Services include care planning, psychological therapy, counselling and support, service navigation and clinical care co-ordination. Services are also available for people in the peri-natal period. Services are offered from a new child friendly hub in Salisbury, or via tele-health. Professional staff match people’s needs with the right care for them using a no wrong door approach.

Paediatric Partnership Project


Adelaide PHN commissions Adelaide Paediatrics to deliver two Paediatric Partnership projects, one in Adelaide’s northern suburbs and one in the central region. The projects work with Northern Adelaide Local Health Network (NALHN) and Women’s and Children’s Health Network (WCHN) Child Development Units to provide Autism Spectrum Disorder, Global Developmental Delay and Psycho-Educational Assessments for children on existing outpatient or assessment waiting lists.

This program only takes referrals from NALHN and WCHN Child Development Units.

Reach and Refer


Adelaide PHN’s Reach and Refer program aims to support general practice to routinely screen for alcohol consumption and intervene when patients (aged over 50 years) need support to address risky or of harmful patterns of alcohol consumption. The program is expected to be delivered over a two-year period and is only available in select general practices across Adelaide. Reach and Refer is using information resources for primary health care that were initially developed by Monash University and which have been adapted by Adelaide PHN.

Refugee and New Arrivals Program


A program designed to improve the health of refugees and new arrivals, build capacity of primary health care workers and increase integration of services for these communities. This program provides refugee health nurses and bilingual, bi-cultural staff to enable individuals and families to connect to the most appropriate medical services. Delivered by Australian Refugee Association (ARA) and Survivors of Torture and Trauma Assistance and Rehabilitation Service (STTARS), this is one of Adelaide PHN’s commissioned services.

Southern Wellbeing Hub - Neami


Free, welcoming mental health service: Individual Psychological Therapy for moderate, complex or severe mental illness ( 10 sessions); Low Intensity individual (3-6 sessions) and group therapies; Suicide Prevention Service; Psychological Therapies for Families and Children up to 11 yrs; Clinical Care Coordination including metabolic checks; Perinatal Program; Mothers and Infants dialectical behavioural therapy groups and other DBT groups; Psychiatric assessments; and longer term individual therapy for people experiencing mod to severe mental illness with complex needs.

  • The Neami Southern Hub mental health services are for individuals 16 years and over and children and families on low incomes, needing different levels of intensity, and lengths of time of mental health support.
  • Referral is via General Practitioners and via self referral generally through the National Head to Health Phone and Digital Intake Service (HtH PDIS). A walk in component will be part of the service, and self referral may be either via telephone on (08) 8326 3591 or via email linkstowellbeing@neaminational.org.au.
  • The Southern Wellbeing Hub – Neami is open from 8:30 to 5pm, Mondays, Tuesdays and Fridays and from 8:30am to 8pm Wednesdays and Thursdays, and every other Saturday morning from 9am to 1pm.
  • At specific times the Hub will open for walk ins, and peer workers will be available to provide information and brief support in a supportive, warm and welcoming environment

Specialised Mental Health Support - for Aboriginal people


Adelaide PHN commissions Nunkuwarrin Yunti to deliver mental health services for Aboriginal people across the Adelaide region. Nunkuwarrin Yunti provides treatment across the severity spectrum (from mild through to more complex conditions). The ACT Centre is also funded by the PHN and provides a clinical therapy service for clients with more complex conditions. This service is embedded within the Aboriginal Community Connect service. A ‘one-stop’ service for those seeking help with drug/alcohol and other problems.

StreetLink - Drug and Alcohol Treatment Services


The Streetlink – Alcohol and Other Drugs (AOD) Treatment Service provides AOD interventions to young people aged ten to twenty-five (10-25) years with complex needs. Streetlink also supports the families of clients who are impacted by problematic substance use. Treatment interventions include peer support (including SMART recovery groups); drop-in services; information and education; screening and interventions; comprehensive assessment; care coordination; withdrawal support; limited pharmacotherapy. This program is one of a suite of AOD treatment services commissioned by Adelaide PHN.

STTARS mental health service for adults from culturally and linguistically diverse communities


A program to improve the mental health of adults from culturally and linguistically diverse communities. Bi-lingual peer support workers will help build individual capacity through providing mental health information and support to navigate and access appropriate mental health services in the community. Culturally sensitive low intensity therapy for individuals and groups and clinical care coordination is available for eligible clients. 

  • The STTARS mental health service for CALD communities is available for adults (18+) from culturally and linguistically diverse backgrounds.
  • It is open from 9am to 5pm, Monday to Friday.

Suicide Prevention Service - supporting those impacted by suicide


Short term support for individuals and families who have experienced a suicide attempt or strong suicidal ideations. The service is provided for free and offers intensive support for 3 months, helping people to develop strategies to prevent further suicide attempts. This service is delivered by AnglicareSA across the Adelaide region as part of a suite of specialised primary mental health services commissioned by Adelaide PHN.

Supporting the Primary Care Response to Family Domestic and Sexual Violence


Adelaide PHN and Country SA PHN in partnership, have received Commonwealth funding to participate in Round 2 of a national pilot to strengthen the capacity of the primary care sector to respond to family, domestic and sexual violence. We are seeking involvement from primary care providers and organisations that provide support to individuals experiencing, or at risk of, FDSV in our needs assessment and co-design processes, so that we can understand where support is needed, and what kind of support will be most suitable.

The SA Immunisation Hub - a program improving vaccine coverage and immunisation awareness


The SA Immunisation Hub works to bridge gaps in immunisation service provision and build capacity of providers via activities including education, mentoring, and network support. The Hub’s team support the Champion Nurse Immunisation Program, the popular Introduction to Immunisation course for new providers, and assist providers in regard to uploading quality data to the Australian Immunisation Register (AIR). The Way Back Support Service


Delivered by AnglicareSA, The Way Back Support Service has been developed by Beyond Blue and is co-funded by the Commonwealth Government and SA Health through Adelaide PHN and the Office of the Chief Psychiatrist. The service has been designed to provide free, non-clinical care and practical support for up to three months for individuals following a suicide attempt or suicidal crisis that have presented to the Royal Adelaide Hospital (RAH) or the Queen Elizabeth Hospital (TQEH).

The Way Back Support Service accepts referrals from the RAH and TQEH with support coordinators providing an outreach service to individuals. Presentations to hospital on weekends or after hours will be followed up via phone call during the next business day.

The service operates between 9.00am – 5.00pm Monday to Friday and can be contacted by calling 1300 077 798 or email thewayback@anglicaresa.com.au. 

The Western Wellbeing Hub by Sonder


The Western Wellbeing Hub by Sonder offers a range of free mental health services to improve wellbeing. Services include psychological therapy, counselling, peer coaching, service navigation, clinical care coordination for people experiencing chronic and complex conditions, parenting groups and clinical suicide prevention services for people who are suitable for a primary care environment. Professional staff will match peoples needs with the right care for them using a no wrong door approach.

  • Referrals received directly or via the Head to Health assessment and phone.
  • This service is run from Monday to Friday, 8 am to 6 pm. Please call on 08 8209 0700 for more information.
  • Mental health treatment plans are required for counselling, structured psychological therapy and clinical care coordination.

Thorne Harbour Health Mental Health Service


A specialist mental health service for lesbian, gay, bisexual, pansexual, transgender, non-binary and gender diverse, intersex, queer, and asexual community members experiencing mental illness and psychological distress aged 16 and above. The service provides peer support, brief intervention, clinical care coordination, counselling and structured psychological therapies. Staff have lived experience and/or extensive experience working with LGBTIQA+ people and offer a trauma-informed, sex-positive, gender and sexuality affirming service.

Wellness Connect - National Psychosocial Support Program


The National Psychosocial Support Program offers one-to-one coaching and group activities that support recovery for people aged 18-64 years with serious mental health who live in the Adelaide metropolitan area and not supported by the NDIS. The program offers three types of support - individual support, NDIS application, and group supports. For further information, please contact 1300 358 220.

Youth Enhanced Service (YES)


The YES program is here to assist young people aged 12-25 years with moderate to high mental health care needs in metropolitan Adelaide. YES provides psychological therapies (individual, family, and group), clinical care coordination, peer work and functional recovery interventions, as well as access to medical care. Service are offered at a central hub in Adelaide’s CBD with place-based (outreach) and digital health services also available.
