Program overview
The Integrated Team Care program is delivered across three sites within the Adelaide region and supports Aboriginal people to better manage chronic health conditions. The program works at the local level by engaging with community, health care providers and Aboriginal organisations. Each service delivery team comprises of clinical Care Coordinators and Aboriginal Outreach Workers working closely together to improve access to culturally responsive multi-disciplinary care (including medical specialists and allied health providers), and building capacity for the effective self-management of chronic conditions.
The program adopts a team-based approach. Outreach workers and care coordinators will support you in accessing medical care and other culturally appropriate health and community services.
Free support for Aboriginal and/or Torres Strait Islander people to better manage chronic conditions, including cancer, diabetes, kidney disease, and/or lung disease.
