Care Connections Good Health - Good Life Project: General Practices Identified to Participate in Project


Adelaide PHN announces that following the expression of interest process, six general practices have been selected to participate in the Care Connections Good Health - Good Life project across the Adelaide metropolitan region. 

The six participating general practices are: 

  • Arkaba Medical Centre 
  • Bayside Family Medical and Musculoskeletal Practice 
  • Ingle Farm Healthcare 
  • Para Hills Clinic 
  • Westwood Medical Centre 
  • Woodcroft Medical Centre 

The Care Connections Good Health – Good Life project aims to assist community dwelling people (60 years and over) to stay well and out of hospital through primary health care improvement and integration activities. 

General practices participating in the Project will undertake several quality improvement activities focused on their patient cohort aged 60 years and over. Quality improvement activities will include risk stratification using digital health innovations, reviewing health assessment and care planning processes and harnessing digital heath technologies to support this cohort. 

In addition, the six general practices will participate in facilitated Communities of Practice, working alongside their local medical neighbourhood. 

Adelaide PHN has commissioned Simple Healthcare Solutions to work alongside Adelaide PHN practice facilitators to support the six general practices participating in the Project.
