General Practice EOI Open: Participate in the Care Connections Good Health Good Life Project

Adelaide PHN invites general practices with a special interest in healthy ageing and/or frailty or have a higher than average older population accessing their practice to consider submitting an Expression of Interest (EOI) to participate in the Care Connections Good Health – Good Life.

The Project aims to assist community-dwelling older people (60 years and older) to stay well and out of hospital through primary care improvement and integration. The intended objectives of the Project are to:

• Encourage and support professional and clinical integration at a local level
• Deliver primary care based healthy ageing and frailty initiatives for community-dwelling older people
• Support quality use of medicines in older people
