Adelaide PHN invites general practices with a special interest in healthy ageing and/or frailty or have a higher than average older population accessing their practice to consider submitting an Expression of Interest (EOI) to participate in the Care Connections – Live Long and Prosper Project.
The Project aims to assist community-dwelling older people (60 years and older) to stay well and out of hospital through primary care improvement and integration. The intended objectives of the Project are to:
- Encourage and support professional and clinical integration at a local level.
- Deliver primary care based healthy ageing and frailty initiatives for community-dwelling older people.
- Support quality use of medicines in older people.
