Adelaide PHN delivers Primary Mental Health Care Services through a network of contracted service providers in the Adelaide region. Adelaide PHN's Primary Mental Health Care Service provides short-term mental health services for people aged 12 years and older who are experiencing mental health issues who have difficulty accessing other services. Adelaide PHN Primary Mental Health Care services are provided at no cost to the client. This brief provides a summary of a report commissioned by the National Association for Medical Deputising (NAMDS) to assess the benefits of after hours doctor visits to patients’ homes and aged care facilities (ACFs). The report, Analysis of After Hours Primary Care Pathways, was produced by Deloitte Access Economics and publicly released on 21 November 2016.
CALHN had over 15 questions during the course of the 30 minute Q&A session, mainly relating to the communications between providers and the patient and specifically relating to the ability to communicate in a timely manner electronically. Here were the key themes from the Q&A session:

The Hip Fracture Care Clinical Care Standard and accompanying resources were launched in September 2016, and provide guidance to consumers, clinicians and health services on delivering appropriate care to people with a hip fracture.

‘My heart, my life’ (MHML) is a secondary prevention resource for patients with coronary heart disease. It provides practical support for their heart condition and care after hospital discharge. Funded by SA Health, it is freely available to primary care services. Cardiac nurses have been distributing MHML to eligible patients in SA hospitals since 2010. We hope that patients who did not receive a copy of MHML in hospital will be given one at their primary care service.
