An RTF referral form for Adelaide’s headspace Early Psychosis Program* suitable for improt into Best Practice clinical software.
This program ensures the provision of early intervention, responsive, and recovery focused care for young people (aged 12-25) who are at high risk of or are experiencing a first episode of psychosis. This is based on the Early Psychosis Prevention and Intervention Centre (EPPIC) model of care.
