The Chronic Disease Support Program is designed to develop the knowledge and skills required to support an individual or client with a chronic disease.
Course delivery
- One day face-to-face workshop
- In-class activities assessed by a qualified assessor
- Workplace activity
- Online assessment completed via BenchmarqueIQ
Suitable for GPs, enrolled and registered nurses, and Aboriginal and/or Torres Strait Islander health workers or practitioners.
